Accident Investigation and Human Factors: A regulator’s perspective

Sam will be sharing some insights based on her experience of investigating dutyholder accidents, as an Inspector in both HSE (non-specialist Inspector) and ONR (Conventional Inspector & HF Specialist Inspector).
Using nuclear and non-nuclear examples of cases she has worked on, Sam will touch on the importance of Human Factors contribution to internal licensee investigation practices when identifying what really went wrong, and the corrective actions necessary to prevent recurrence, highlighting commonly observed challenges which dutyholders sometime face.
The session will provide an opportunity to network, share ideas and experiences associated with HF. All members welcome, whether you work in nuclear or not!
