Human Failures and Safety Critical Task Analysis (Risktec)

Provides the necessary baseline knowledge and skills to be able to start undertaking Safety Critical Task Analysis.

The course is designed primarily for staff working for companies in the UK oil & gas & chemicals sector who have a regulatory requirement to carry out Safety Critical Task Analysis (SCTA), although the principles are also applicable to other safety sectors where SCTA may be appropriate.

The course is designed primarily for staff working for companies in the UK oil & gas and chemicals sector who have a regulatory requirement to carry out SCTA, although the principles are also applicable to other safety sectors where SCTA may be appropriate. The course is targeted at anyone who may be required to plan, facilitate and/or take part in SCTA, including Health & Safety Managers, Process Safety specialists, training managers, supervisors, plant operators and maintainers.

Why should you attend?

The course provides the opportunity to obtain practical guidance on the SCTA process from experienced Human Factors consultants who have successfully delivered SCTA many times and can advise on both the pitfalls and the process to be followed to ensure regulatory sign-off. The course is designed to be the first step in building in-house competence in SCTA, and to enable organisations to act as an ‘intelligent customer’ when procuring specialist SCTA support.

What will you learn?

You will be taught how to undertake the full SCTA process, including how to carry out safety critical activity screening and detailed Human Failure Assessment (HFA). To support this process, you will learn about human failures, the Performance Influencing Factors (PIFs) which can affect their occurrence, and how these PIFs can be mitigated.

Expected outcomes

At the end of the course, you will:

  • Be able to define what is meant by Human Factors.
  • Understand the key legislation and guidance that is relevant to the application of Human Factors within the UK oil & gas sector.
  • Understand how human failures can contribute to accidents, and be able to identify credible human errors and violations.
  • Be able to identify PIFs that can make human failures more likely to occur, and understand how systems, tasks and processes can be designed to minimise their effects.
  • Understand the process to be followed for human failure identification and assessment, and the different approaches that are available.
  • Understand in detail the steps involved in the SCTA process.
  • Have gained experience in working through the SCTA process, including initial screening of activities and detailed HFA.

Assessment

There is a multiple-choice assessment at the end of the course.

The course includes a number of interactive group exercises, including safety critical activity screening and detailed HFA using worked examples.
